Forms must be filled out at time of booking with the exception of Covid-19 Waiver which must be filled out within 24 hours of appointment.

I hold harmless Hope Hypnosis & Holistic Center LLC t/a Hope Institute of Healing Arts, its

principals and subcontractors from any liability in regards to taking this class. I understand that

there may be some energy healing touch involved within the class in practice sessions,

placements, ignitions and attunements. Deposits are not refundable and a new deposit after the first rescheduled appointment will be requested.  No refunds
